Total Value of Goods in Transit
This is the combined value of all the goods you were contracted to move, not just the ones that you are claiming for.
Estimated Claim Value
This is a rough estimation of the combined value of the goods you are claiming for, please note if the item is repairable then the repair cost should be the price used.
Do you accept liability for the claim against you
Here we need your comments on whether you accept liability for the damage/loss casued.
Evidence of Value
In order to process your claim, we will require Proof of Purchase for the item(s) you are claiming for, if this is not available we will require the Purchase date.
